Registration Form
Download the registration form for instructions and tournament information.
You can either fill out this form online, or mail in a printed form. Please make all checks payable to Evergreen Missionary Church.
If you fill out the form online, you can choose to pay using paypal. There is 3% charge for doing this, because paypal charges us 3%. You can either pay using a paypal account or with a creditcard. You will see a button when your registration form has been processed, directing you to the paypal account.
Even if you register online, you may still mail us a check.
